Executive Summary: Following a miscommunication with tower, in very poor visibility and in a congested airport ground environment, a KLM 747 began its takeoff roll while a Pan Am 747 was taxiing on the runway. The KLM attempted an evasive maneuver, but was destroyed along with all occupants. The Pan Am had significant casualties.
